We invite you to complete the attached 9/11 10th Anniversary Events/Memorial Service Submission Form to help identify events that 9/11 family members, recovery personnel or the public can attend
this year. The information you provide will be made available to 9/11 family members and recovery personnel as well as the general public based on your indicating what information should be
accessed by which population. As of March, an online version of the compiled information will be made available on the 9/11 Healing and Remembrance website at:

This 9/11 Anniversary Events project is being conducted as part of the 9/11 Healing & Remembrance Program, administered by the Mental Health Association of New York City (MHA-NYC) through a grant
from the U.S. Department of Justice, Office of Victims of Crime.
The submission intake process was prepared by the National Disaster Interfaiths Network. This is an open submission process. Please submit all events as soon as possible so that families and organizations can best plan their
schedules and avoid conflicts.
