Volunteer Group Housing Form-Set

New York Disaster Interfaith Services' (NYDIS) Volunteer Group Housing Program is pleased to release its Volunteer Group Housing Form-Set. With a special concentration on religious and cultural needs, these handbooks and forms will provide organizations with the knowledge and resources necessary to begin and sustain a long-term volunteer housing program. These documents include formative advice based on sector best practices, partnerships with existing volunteer housing programs, and input from the NYC Volunteer Group Housing Task Force.
FORM-SET HANDBOOKS
Host Site Handbook
Gives organizations a comprehensive introduction to volunteer housing and the knowledge needed to begin or sustain a successful housing program.
Additionally this handbook includes, the following attachments: Writing A Host Site Manual, Facility Survey, Good Faith Agreement, Volunteer Group Form, Individual Volunteer Form, Sample Packing List and the Individual Indemnity and Waiver of Liability Form. The entire handbook can be downloaded as one or the attachments downloaded separately.

Volunteer Group Handbook
A compendium to the Host Site Handbook to be distributed to incoming volunteer groups by host sites. It offers volunteer groups a comprehensive introduction to volunteer housing, and the perspective needed to have a successful service project. While any volunteer group will find this handbook informing, volunteer groups working in NYC on repair and rebuild projects in response to SuperStorm Sandy are the primary audience.
Additionally it includes, the following attachments: Facility Survey, Good Faith Agreement, Volunteer Group Form, Individual Volunteer Form, Sample Packing List. The entire handbook can be downloaded as one or the attachments downloaded separately.

Writing a Host Site Manual
Guides host sites through how to compose a Host Site Manual. Produced by a host site, a Host Site Manual is a comprehensive guide made for volunteer groups that provides them with information about the host site facility and community in which they will be volunteering and living. Laundry, meals, sleeping arrangements, and climate should all be discussed in the resultant document.
Additionally this manual includes, the following attachments: Facility Survey, Sample Packing List. The entire manual can be downloaded as one or the attachments downloaded separately.


While the specifics of these documents are tailored for New York City, they can be used anywhere there is a need for volunteers or volunteer group housing. With written permission and appropriate attributions it can be adapted by any disaster impacted community, free of charge.

ATTACHMENTS, FORMS, AND RESOURCES
Facility Survey - This form offers a comprehensive, culturally and religiously competent template to be used by a coordinating agency or host site in assessing a facilities capabilities, capacity, and safety for hosting volunteers.

Good Faith Agreement
- This document is a good faith agreement template between the Host Site and the Volunteer Group. It details the intent and reasonable expectations of both parties, and is designed to be monitored by a third party.

Volunteer Group Form - The form provides vital information about the volunteer group required by the host site for logistics, communication and safety purposes.

Individual Volunteer Form - This form provides essential information about the volunteers required by the host site for logistics, communication and safety purposes.

Sample Packing List - This list should serve as a guide for Volunteer Groups on what each volunteer or the group itself should pack and bring (or avoid void bringing) on their service trip.

Individual Indemnity and Waiver of Liability - The form offer waives any potential liability claims that can be levelled against the Host Site by visiting Volunteer Groups. Host Sites are strongly encouraged to have this form completed by each Volunteer Groups prior to arrival.
